The questionnaire is in four parts and asks the following:

  • your regular or current drinking habits
  • which circumstances you are likely to drink
  • how does drinking affect other areas of your life
  • What do you want to change about your drinking, and why

You’ll get the most from this Pep if you answer the questions thoroughly and honestly. Completing every question will enable me to offer the best advice about stopping safely.

About your drinking
In the last seven days, did you drink more or less alcohol than you usually would?
Do you regularly drink more than 1.5 x bottles of wine, half a bottle of spirits or six pints of beer daily?
Do you need a drink to relieve shakes or sweats soon after you wake up?
Have you ever experienced withdrawal symptoms (like shaking, sweating, anxiety or nausea) when reducing or stopping drinking?
